Focus Community Care provides Assist – Life Stage Transition supports to help NDIS participants manage important changes in their lives. These transitions may include moving to independent living, changes in education or work, developing new routines, or building skills for everyday life.
Our goal is to support you through change in a way that feels safe, planned, and empowering.
Life stage transition support focuses on building skills and confidence to handle daily life challenges and new situations. These supports help participants increase independence, adapt to change, and develop practical life skills needed for everyday living.
We work closely with participants, families, and carers to ensure supports align with personal goals and abilities.
We provide training and guidance to help participants develop everyday life skills that support long-term independence. Training can be delivered one-on-one or through group activities and may focus on:
Personal hygiene and self-care
Household tasks and daily routines
Communication with service providers
Planning travel and using public transport
Problem-solving and decision-making skills
Our Assist – Life Stage Transition supports may include:
Building daily living and independence skills
Support with personal care and hygiene routines
Meal planning, cooking, and nutrition skills
Maintaining a clean and organised home
Developing communication and social skills
Support with transport and public travel skills
Budgeting and basic financial skills
Support during education, work, or housing transitions
All supports are tailored to your NDIS plan and individual needs.
We understand that families and carers play an important role during life transitions. Our team works alongside parents and carers to provide guidance, strategies, and practical support to manage daily challenges and future planning.
This may include support during school transitions, moving into independent or shared living, or managing changes in routine.
At Focus Community Care, we take a respectful and patient approach. We focus on building skills gradually, encouraging confidence, and supporting participants at their own pace. If you have a specific goal or skill you want to develop, we work with you to help achieve it.
If you or your family member needs support during life changes or skill development, Focus Community Care is here to help.
